Important Information to Consider
Membership Plan Terms & Important Information
Membership Is Not Dental Insurance
The Smile Sprouts Kids Dental Membership Plan is not dental insurance and is not a qualified health plan under the Affordable Care Act. Membership benefits are available only through Smile Sprouts Kids Dental and are subject to the terms and limitations of the selected membership tier.
12-Month Membership
Seed, Sprout, and Bloom are 12-month memberships. Members may choose to pay for their membership in full at enrollment or, where available, through a 12-month installment payment option.
Monthly installment payments do not create a month-to-month membership. Members remain responsible for the full 12-month membership commitment, subject to the cancellation and refund provisions below.
Membership payments must remain current for included services, discounts, and other benefits to remain active.
Included Preventive Care
Included services vary by membership tier and may include professional cleanings, doctor examinations, necessary diagnostic X-rays, fluoride treatments, and emergency visits.
Services are subject to clinical necessity and the professional judgment of the treating dentist. Included services must be used during the applicable 12-month membership period. Unused services expire at the end of the membership term and have no cash value.
Treatment Discount
Active members receive 15% off eligible treatment performed at Smile Sprouts Kids Dental that is not otherwise included in the membership.
The membership discount does not apply to orthodontic treatment, services provided by outside specialists or third parties, or other excluded services identified by the practice.
Membership discounts may not be combined with dental insurance, other discounts, promotional pricing, or special offers.
Sibling Membership Discount
Additional eligible siblings enrolled in a Smile Sprouts membership receive 10% off their applicable membership price while the qualifying memberships remain active.
Emergency Exams
Emergency visits included with a membership provide an evaluation by Smile Sprouts Kids Dental for an urgent dental concern. Treatment performed as a result of an emergency examination is not included unless specifically listed as a membership benefit and may be subject to the applicable membership treatment discount.
Seed includes up to 2 emergency visits per membership year.
Sprout includes up to 3 emergency visits per membership year.
Bloom includes up to 4 emergency visits per membership year.
Emergency benefits do not guarantee immediate treatment or replace emergency medical services when appropriate.
Priority Scheduling
Sprout members receive priority access to available weekday appointments.
Bloom members receive priority access to available weekday and designated weekend appointments.
Priority scheduling means preferential access to available appointment times and does not guarantee same-day care, a specific appointment time, or appointment availability outside the practice's operating schedule. Clinical urgency may take priority over membership status.
Bloom Emergency Access
Bloom membership includes access to the doctor's designated private contact number for urgent dental concerns.
This benefit is intended for appropriate dental emergencies and does not guarantee 24-hour immediate response, diagnosis, or treatment. For a life-threatening or medical emergency, call 911 or seek appropriate emergency medical care.
30-Day Money-Back Guarantee
Memberships may be cancelled for a full refund within 30 days of enrollment provided that no included service, membership benefit, product, or membership discount has been used.
Once any membership service, benefit, product, or discount has been used, the membership is no longer eligible for the 30-day money-back guarantee.
Services Not Included
Treatment not specifically included in the selected membership tier is charged separately. Payment for treatment outside the membership is due according to Smile Sprouts Kids Dental's standard financial policies.
Membership benefits do not cover treatment performed by specialists or other providers outside Smile Sprouts Kids Dental.
Dental Insurance
Membership benefits and discounts cannot be combined with dental insurance. Services received using membership benefits will not also receive membership discounts in addition to insurance benefits.
Home-Care Products
Any home-care products provided as part of a membership are subject to the applicable manufacturer's warranty, return, replacement, and shipping policies.
Automatic Renewal
Membership automatically renews for an additional 12-month term unless automatic renewal is turned off before the renewal date.
Members may opt out of automatic renewal at any time through the Member Portal. An email reminder will be sent approximately 45 days before the scheduled renewal date.
Turning off automatic renewal prevents the next membership term from beginning; it does not cancel or terminate the current 12-month membership.
Additional Terms
Membership benefits are non-transferable and have no cash value. Benefits apply only to the enrolled child.
Smile Sprouts Kids Dental reserves the right to update membership pricing, benefits, and terms for future membership periods. Changes will not alter the benefits of an active membership term except as permitted by the Membership Agreement and applicable law.
